Please fill out one form per student with responses to all required prompts. Please be sure to add (if sending a check) or select (if paying via PayPal) the appropriate $35 annual registration fee for an individual student or $55 per family. Please see tuition information under the Tuition tab on the CSDA website. Please submit this registration form and first payment--*including registration fee*- by August 1st. To calculate tuition payment, please add up the number of weekly class hours (40-60 minutes) for each dancer and calculate the payment due for each student. Note that any class (except for TuTu Cute 2s) that shows .5 hours in the left column is for students enrolled in “half classes” (half-hour: Classical Ballet/Pointe, Tap, etc.).


Payment is available by check or online via PayPal/credit card. PayPal buttons (select the one for your number of classes) are under the Tuition tab.  A 3.5% PayPal service fee is included in online payment amounts. For either payment method, please add (if sending a check) or select (if paying via PayPal) the appropriate *$35 annual registration fee for an individual student or $55 per family*. Please be sure to send your PayPal tuition payments directly via the form on the "PayPal Buttons" page of the CSDA website, NOT via your personal PayPal.


For either payment method, tuition may be paid in full or in two installments, due on August 1st and October 1st. Monthly payments may be arranged through director. Please make your check payable to:  Candace Sheppard or CSDA and mail to 4000 Massachusetts Ave., Apt. 1130, Washington DC 20016.  Registration will be accepted in the order received and no further notification will be given unless a class is unavailable.


A $35.00 FEE WILL BE CHARGED FOR ALL RETURNED CHECKS.  NO REFUNDS WILL BE GIVEN FOR MISSED CLASSES OR CLASSES DROPPED AFTER THE FIRST WEEK. MAKE UP CLASSES MUST BE MADE UP WITHIN THE MONTH AND ARRANGED WITH THE TEACHER IF SPACE IS AVAILABLE IN THE CLASS.
